Requests for Information (RFIs) and Requests for Assistance (RFAs)
DOD employees and military personnel will submit RFIs/RFAs to the DHA J-33 using the Enterprise Task Management Software Solution (ETMS2). The DHA Form 435, “Defense Health Agency Operations Center Request for Information/Request for Assistance” is the official method to transmit formal inquiries and requests.

Download and complete DHA Form 435 following the form directions.

Fields 1-3, and 11-14 must be filled out.

Complete fields 4-10 as applicable.
- Submit the completed DHA Form 435 using ETMS2 to the Request for Information team: DHA-RFI_RFA
For those without ETMS2 access or if ETMS2 is down*, e-mail the completed DHA Form 435, as an attachment, to
[email protected].

Fields 1-3 and 11-14 must be filled out.
* When ETMS2 connectivity is restored, the requestor will enter the e-mail content into an ETMS2 tasker for visibility and records management.
